Sanctuary Groups, LLC Waiver 

In consideration of participation in programs and other activities at or sponsored by Sanctuary Groups, LLC and further in consideration of entering and using the facilities owned, leased or otherwise provided by Brandi Diamond and/or John Diamond, I fully agree that I am participating of my own free will.*

Furthermore, fully and completely release, discharge and hold harmless Sanctuary Groups, LLC, as well as Brandi and/or John Diamond, and any directors, trustees, officers, employees, agents, insurers, instructors, coaches, caregivers, and volunteers of the aforementioned from any and all claims, actions, causes of action, and damages of any kind whatsoever, including but not limited to claims for personal injury or property damage. *

As the undersigned, I acknowledge and recognize there are inherent risks involved in certain activities or recreational activities as part of Sanctuary Groups, LLC and I personally assume the risk of any injury sustained while participating in a Sanctuary Groups, LLC event or gathering of any kind.*

As the undersigned, I agree to indemnify, defend, and hold harmless Sanctuary Groups, LLC and its agents and employees [including Brandi and/or John Diamond] from any and all claims arising out of my participation in any event or gathering of Sanctuary Groups, LLC, even if such claim arises as a result of a negligent act or omission of Sanctuary Groups, LLC, or Brandi and/or John Diamond.*

I give consent for my child to be photographed, videotaped, or filmed while participating in class activities and for the resulting images to be used by Sanctuary Groups, LLC or Brandi Diamond.*

I agree that by submitting this form I am represented by and through this agreement as a digital signature, and agree that doing so represents me both legally and otherwise. *
